Each
patient will receive an individualized treatment plan
designed for optimal recovery. Our focus is on increasing
functional independence and sending patients home or back to
work as soon as possible. Weekly team meetings are held to
review the care plan of the patient based on his/her
progress and to evaluate the treatment plan for any needed
adjustments.
Once the
goals have been reached, or the team determines the patient
has maximized his/her treatment potential, the patient will
be discharged to his/her home or to an environment that will
best meet his/her needs.
Extended Care Programs:
Our
Extended Care Programs combine around-the-clock health
care with the comforts of living at home. Our primary focus
for the resident’s stay is to enhance the quality of life by
providing a nurturing environment that helps our residents
to stay active and healthy.

Medicare Information/Skilled Nursing Benefits
To be
eligible for a Medicare skilled nursing stay, patients must
have Medicare Part A benefits and have been hospitalized for
three (3) consecutive days within the last thirty (30) days.
Medicare
coverage is established by federal guidelines.
A patient
is allowed up to 100 days of skilled coverage per benefit
period, if all eligibility and medical criteria are met.
Medicare will pay all of the medical charges (including room
and meals) for the first twenty (20) days and only partially
for the remaining eighty (80) days. The “co-insurance” for
these 80 days is the financial responsibility of the
patient/responsible party.
Medicare will not cover personal convenience items such as
the additional cost of a private room, barber/beauty
services, private telephone or cable television.
The
patient and responsible parties are encouraged to
participate in weekly care conferences. Once the patient
reaches his/her goals, or the team determines he/she is no
longer progressing through the treatment program, the
patient will be discharged from the skilled nursing program. |
